For the first time in years, Tracey Phillips is celebrating the holidays in a safe, supportive home of her own. After receiving expert long-term care from Broadway House for Continuing Care, she was discharged earlier this year and moved into an apartment at HELP Genesis JBJ Soul Homes in Newark, a 51-unit affordable housing complex located a few blocks from Broadway House.

Approximately a quarter of apartments in the complex are occupied by tenants living with HIV/AIDS. The complex provides its residents with in-house support services, including case management and employment assistance. Many current tenants were referred by Broadway House, which collaborates with the complex to arrange housing for our discharged residents.

Broadway House Medical Case Manager Maria Lorenzo is part of the team that supports residents like Tracey as they prepare to begin their lives anew in the community. Ms. Lorenzo’s role is critical for long-term success: “It takes a lot of coordination when a resident is returned to the community. Yet, it’s worth every bit of the effort.”

Once homeless and battling substance abuse, Tracey spent several years at Broadway House benefiting from expert medical care, as well as extensive behavioral and social support. Her care team ensured that she received substantial physical rehabilitation to recover from knee surgery, as well as support to bring her substance abuse, diabetes, and high blood pressure under control.

Now, Tracey is hoping to enroll in a job skills program for women to gain more confidence and help provide for herself: “I’m happy to have a real place to call home. My health is good, and that makes my mother so happy. Her prayers were answered. If it weren’t for Broadway House, I don’t know where I’d be.”
